The cause of death of Blake Garrett from “How to Eat Fried Worms” has been revealed.
The former child star died on February 8 at the age of 33 from an accidental fentanyl overdose, according to a report from the Oklahoma State Office of the Chief Medical Examiner obtained by TMZ on Monday.
According to reports, he lived in a modest living home in Tulsa, Oklahoma.
Garrett was rushed to the hospital a week before his death and was diagnosed with shingles, his mother Carol previously told the show.
At the time, the matriarch feared her son’s death was accidental and speculated that he had been self-medicating.
The CDC defines shingles as a “painful rash” that is “caused by the varicella-zoster virus (VZV), the same virus that causes chickenpox.”
Garrett starred as Plug in the 2006 Bob Dolman film How to Eat Fried Worms, alongside actors Luke Benward, Harry Eisenberg, Adam Hicks, Ryan Margarini, and Alexander Gould.
The film is based on the 1973 book of the same name by Thomas Rockwell.
Garrett played the bully’s sidekick in the film, but in a 2006 interview with The Oklahoman, Garrett reassured that all of his castmates were friends off-screen.
“Everyone got along well on set,” he said.
The film’s cast won Best Young Ensemble Feature Film at the 2007 Young Artist Awards.
Additionally, Garrett appeared in the arena show production “Barney’s Colorful World International Tour” from 2003 to 2004.
He also appeared in many local theater productions, including “Grease,” “Aladdin and the Magic Lamp,” “The Wizard of Oz,” “Annie,” and “Peanuts: A Charlie Brown Tribute.”
